Wow! I've never heard of this vidui. Where did you find it?

I'm still not positive but I am seriously leaning toward thinking that this was inspired but not written by Rav Kook. it's based on his take of vidui ma'aser...

Sorry for any misunderstanding.

For the record, R' Kook didn't write this "vidui". It was written by (Rabbi) Binyamin Holtzman whose name is on the side of the image, with inspiration from R' Kook's quote at the bottom.

The full R' Kook quote, translated, is as follows:
"Just as there is a great value in improving the soul by confessing sins, there is also a value in periodically - indeed less often and constantly that the confession of sins, so that his heart will not be moved and bring him to laziness and pride and frivolity, but occasionally, the righteous servants of God will also find value in confessing merits, so that they may take joy in them and strengthen the ways of God in his life..."
